Members of the Solstice S500A dental plan are eligible to receive Benefits immediately upon the effective date of coverage with: • No Benefit Waiting Periods • No Deductibles • No Claim Forms to Submit The Member Copayments listed are offered by a Participating Provider. The Member receives: • Most diagnostic and preventive care at no charge • Cosmetic and orthodontia treatment Covered The Member is ultimately responsible for verifications to the accuracy and appropriateness of all fees applicable to any dental benefit provided by a Network Provider. We urge all of our Members to verify all fees for proposed treatment via the Schedule of Benefits and/or with our Member Services Department prior to treatment.
